I have a master’s degree in social work and I completed the required hours of training through an internship where I provided counsel for patients under the supervision of a professional. I am certified and I have a license to practice in this state. I have the experience and training needed to provide services to patients with serious and sometimes terminal illnesses and diseases.

I have the ability to assist other professionals with the discharge process by explaining policies and procedures to the patients and their families. I can also help patients make arrangements for in-home care or arrange for them to move to a different facility if necessary. I can also provide them with resources that can help them get the care they need.

I possess excellent problem solving and decision-making skills along with the ability to manage my time wisely. I can keep all documents and patient records organized and updated. I also have the means to travel to different areas to visit discharged patients to follow up and see how they are doing.
